Please plan to arrive 20 minutes prior to your appointment to complete your paper work. If you prefer, you may also send us your paperwork on-line using our secure internet connection.

Insurance

If you have insurance, please bring your card with you on the day of your appointment. Or if you would like, you may fax us a copy of your card, both the front and back, and enlarged if possible.
